Pre-Registration For COVID-19 Testing

You will get a QR code after you finish the registration process. Please show the on-site staff the QR code for them to start your visit.

If you are experiencing severe symptoms such as extreme shortness of breath, contact your physician. If it is an emergency, dial 911.

You do not need an appointment and no pre-registration is required but is encouraged to save you time.
Upon arrival, your credentials will be verified, and our clinical staff will ask you questions to ensure it is safe to administer the vaccine.
For pre-registered patients, our staff will scan your pre-registration QR code. For drive-ups, you will be asked to register on our patient portal, Intend/DragonflyPHD. Our staff is there to assist in the registration process.
Intend/DragonflyPHD is a HIPAA-compliant platform that is used to electronically maintain and deliver your information securely and privately.
Please have your I.D. / driver’s license or Passport and insurance information (if you have insurance) ready when it’s your turn.
If you have an existing CDC vaccination card, please be sure to bring it with you!

Remain in your car, lower your window and our medical staff will discuss and administer your vaccination.
The vaccination process should take approximately 10 minutes. You may be asked to wait 15-30 minutes so our trained personnel may observe for adverse reactions
